Before any home care is put in place, part of our bespoke home care service involves a free, at-home care needs assessment. The purpose of the assessment is to record and understand the client’s needs, wishes and preferences so that we can deliver care tailored to their lifestyle.
The outcome of the care needs assessment is used to inform the client’s bespoke care plan. The care plan details what the client needs and wants and how this will be delivered. The care plan is a living document that is used by care professionals and updated regularly to reflect the client’s bespoke care needs, wishes and preferences.
The care plan and other healthcare professionals involved in a person’s care work with the client to identify the most suitable type of bespoke home care that fits their needs and wishes.
