Pam, 99, had been living at home with minimal support when she developed a serious, ungraded pressure sore. Over several months, district nurse visits were inconsistent, leaving the wound untreated. Her daughter, who lived next door, was unaware of the severity. After a fall and hospital admission due to infection, the hospital recommended permanent care home placement. Pam was able to mobilise only with a Zimmer frame and could not manage the stairs in her semi-detached home. Despite this, she was determined to return home.
Her daughter contacted us for support. Within two days, we assessed both her care home placement and home environment. The care manager proposed creating a “micro-environment” in the dining area and liaised with the hospital occupational therapist to arrange equipment and support. Pam was fully involved in the plan, including the long-term goal of returning to her bedroom upstairs.
Just four days after assessment, Pam returned home with a live-in carer matched to her personality and needs. Her care team worked closely with the Tissue Viability Nurse, district nurses, and physiotherapist to implement structured repositioning, wound care, and mobility exercises.
Pam set a clear goal: to regain independence and sleep in her own bed upstairs. Her family supported her determination, installing a stair lift as a precaution. Over six months, she followed the physiotherapy plan, gradually improving strength and mobility. Her pressure sore healed completely, and she regained the ability to walk upstairs with supervision.
Her daughter said:
