A client had been receiving companionship and shopping support from Trinity Homecare for more than two years. These visits had become an important part of their weekly routine, enabling them to remain independent, access their local community and continue participating in activities that mattered to them.
Each week, the client and their carer would visit a local supermarket, often stopping for a coffee or something to eat before completing their shopping. The visits provided far more than practical support. They promoted social interaction, maintained independence and contributed positively to emotional wellbeing and quality of life.
On this occasion, the visit began as it always had. After arriving at the supermarket, the carer parked the vehicle and exited briefly to connect it to a charging point. Before leaving the car, she clearly explained to the client that they should remain seated and reassured them that she would return shortly.
Despite these instructions, the client independently exited the vehicle using their walking aid. While attempting to mobilise, they lost their balance and fell backwards, striking the back of their head and sustaining an injury.
Recognising the seriousness of the incident, the carer acted immediately. She contacted emergency services and notified the office, providing clear and timely information regarding the client’s condition. Although understandably distressed, she remained with the client, ensuring they were not left alone and continuing to provide reassurance while waiting for further support.
The Care Manager and Registered Manager attended the scene without delay to provide additional assistance. On arrival, they administered first aid and remained with the client throughout the incident, ensuring they remained as comfortable as possible while awaiting the ambulance. They also recognised the significant emotional impact the event had on the carer and provided reassurance and support during what was an extremely distressing situation.
The supermarket management team also demonstrated excellent partnership working, offering practical support throughout the incident.
The professionalism demonstrated by the team was recognised externally. Following the incident, the supermarket’s Duty Manager contacted Trinity to express their appreciation for the response provided, commenting that they were pleased to hear the client had returned home safely and commending the team for their actions during a difficult situation.
This independent feedback reinforces the calm, compassionate and professional manner in which the team managed an extremely challenging situation, ensuring the client remained supported throughout while also demonstrating effective leadership and partnership working within the wider community.
As time passed, the client’s condition began to deteriorate. Their breathing became increasingly laboured and they reported worsening pain throughout their body. Recognising these as potentially significant changes, the management team repeatedly contacted the ambulance service to provide updates and ensure emergency responders were aware of the escalating concerns.
Given the changes in the client’s presentation, the ambulance call handler instructed the team to obtain a defibrillator as a precautionary measure while continuing to monitor their condition. Despite waiting more than an hour for emergency services to arrive, the team remained with the client throughout, ensuring they were never alone during this frightening experience.
Upon arrival, the ambulance crew carried out a full assessment before transferring the client to hospital for further treatment.
While the outcome of the incident was beyond the control of the service, this case demonstrates the critical role domiciliary care teams play in responding to unexpected emergencies within the community. The professionalism displayed by the carer, coupled with the rapid escalation and leadership support provided by the management team, ensured the client received timely and compassionate care during a period of significant vulnerability.
The case also highlighted the importance of supporting staff following traumatic events. Recognising the emotional impact on the carer, managers provided immediate reassurance and follow up support, reinforcing Trinity’s commitment to both the people it supports and the colleagues who deliver care.
This case demonstrates that outstanding care is not only measured by the outcomes achieved but also by how services respond when things do not go as planned. Through calm decision-making, effective communication and compassionate leadership, Trinity ensured the client received the dignity, support and advocacy they needed during an unexpected medical emergency.
